An 8-year old spayed female mixed breed dog was presented to the Cornell University Hospital for Animals (CUHA) Emergency Service for evaluation of thrombocytopenia, systemic hypertension, bilateral hyphema and glaucoma. The dog was serologically positive for antibodies to Borrelia burgdoiferi. In addition to the findings of her referring veterinarian, her physical exam revealed lymphadenopathy, mydriasis and an absent menace response bilaterally, petechia on the ventral abdomen and jugular pulsation. Ocular ultrasound lead to suspicion of retinal detachment in the left eye. Thoracic radio graphs and abdominal ultrasound were absent of signs of metastasis. The patient was discharged to her owner on systemic mycophenolate, prednisone and doxycycline and ophthalmic prednisone acetate, dorzolamide/timolol, atropine and Paralube. After her discharge, her anti-nuclear antibody (ANA) test was determined to be increased consistent with a definitive diagnosis of systemic lupus erythematosus. Since discharge, she has continued immunosuppressive therapy and has regained vision. Her intraocular pressures and platelet count have normalized.
