A six month old mixed breed dog presented for evaluation of ascites and a grade V/VI cardiac murmur. Echocardiography revealed severe hypertrophy of the right ventricle, severe pulmonic stenosis, and severe tricuspid regurgitation. A balloon valvuloplasty procedure was performed. Vascular access was obtained via the right femoral vein and left jugular vein. Transesophageal echocardiography was performed to measure the diameter of the pulmonary valve annulus. A balloon that was approximately 1.3 times the diameter of the pulmonic valve annulus was selected and inflated at the sight of the stenosis, causing dilation of the annulus. The goal of this procedure is to reduce the transstenotic pressure gradient by half, which was achieved immediately post-procedure in this case. The clinical signs resolved, but at the two month follow-up visit, the pressure gradient showed only minor improvement from its original value.
