Abstract

A two-year-old male castrated Labrador Retriever presented to the Cornell Emergency Service for one episode of vomiting and elevated liver enzymes. The patient was admitted and maintained on intravenous fluids, anti-emetics and liver protectants prior to transfer to the Internal Medicine Service the next day. Blood work revealed increased ALT, AST, GGT and Bilirubin. Urinalysis revealed proteinuria and glucosuria. Physical exam revealed a soft, non-painful abdomen with no masses palpated. Abdominal ultrasound revealed a diffusely small and mottled liver, with no evidence of portal hypertension or acquired shunts. The patient was taken to surgery to obtain liver biopsies that were submitted for cultures, histopathology and liver copper, iron and zinc levels. Histopathology and copper levels were consistent with a copper-associated hepatitis. The patient was discharged with antibiotics, gastroprotectants and liver protectants. The patient was also started on a low copper diet and D-penicillamine therapy for chelation of excess copper.
